A mini breast lift — also known as a doughnut mastopexy or periareolar mastopexy — is a breast lift technique using a single circumferential incision around the areola to elevate the nipple-areola complex and remove a modest amount of excess skin. It is the least breast lift technique, producing the smallest scar burden, and is appropriate for patients with mild degrees of breast ptosis.

The doughnut lift is for patients who need a slight elevation of the nipple and modest skin tightening, but do not have the degree of ptosis that would require the more incisions of a or anchor lift. In cases of mild sagging, it can often achieve comparable to a full lift with significantly less scarring.

A periareolar mastopexy is commonly combined with breast augmentation — an implant can be placed through the same periareolar incision in the same session, allowing simultaneous correction of nipple position and volume. It can also be performed as a standalone procedure or as part of a mummy makeover.

At Centre for Surgery, mini breast lift surgery is performed by consultant plastic surgeons on the GMC Specialist at our CQC-regulated Baker Street facility under TIVA (Total Intravenous Anaesthesia) as a day-case. A two-week cooling-off period applies after consultation.

How Does a Doughnut Lift Differ from a Full Breast Lift?

A doughnut mastopexy differs from a traditional full breast lift in the incision pattern, extent of skin removal, and degree of correction achievable.

A doughnut breast lift involves two circumferential incisions around the areola. The skin between them is removed and the incisions closed with dissolvable suturesresulting in a single periareolar scar. The elevates the nipple to a higher and removes a modest amount of excess skin from around the areola.

A full breast lift — either (vertical) or anchor (inverted T) — uses additional vertical and/or horizontal incisions to allow greater skin removal and more comprehensive tissue reshaping. This is appropriate for moderate to severe ptosis.

The key differences:

Breast implant alone may be able to achieve a similar apparent improvement to a doughnut breast lift for patients with very mild ptosis — without any periareolar incision. Your surgeon will assess whether implants alone, a doughnut lift alone, or a combined approach is most appropriate for your anatomy at consultation.

Am I a Good Candidate for a Mini Breast Lift?

Doughnut mastopexy is appropriate for patients with mild to breast ptosis who do not require the more extensive correction of a full breast lift. Your surgeon will carefully listen to your needs and desires before conducting a comprehensive physical examination, taking several factors into account. This will include the degree of breast sagging, the amount of loose skin, and the current position of the nipple and areola complex in relation to the lower breast crease.

A doughnut lift is an ideal procedure in the following situations:

Women who do not need or want more breast lift surgery may be able to achieve great results with a periareolar mastopexy. It is essential to bear in mind that breast implant surgery alone may be able to achieve similar results to a doughnut breast lift without incisions and scars around the nipples.

What Does Mini Breast Lift Surgery Involve?

A periareolar mastopexy is performed as a day-case at our Baker Street facility under TIVA (Total Anaesthesia) — the safest form of general anaesthesia for day-case . The procedure takes approximately 90 minutes for a standalone doughnut lift; longer if combined with breast augmentation or other procedures.

Two circumferential incisions are made around the areola — an inner incision at the areola border and a larger outer incision in the surrounding breast skin. The skin between the two incisions is surgically removed. In some cases, a small amount of breast tissue may also be removed to optimise breast shape.

The incisions are closed with dissolvable sutures using a technique — which helps prevent the areola from enlarging over time, a known limitation of periareolar closures.

If breast augmentation is being in the same session, the implant is placed through the same periareolar incision before closure.

All are closed with dissolvable sutures only — no suture removal appointment needed. A post-surgical bra is applied before discharge.

After the procedure, you recover in our monitored day suite before discharge with your responsible adult, post-operative medications, written care instructions, and a direct 24/7 clinical support number.

A doughnut mastopexy is combined with breast augmentationallowing simultaneous correction of nipple position, mild ptosis, and volume in a single operation. It can also be combined with other procedures as part of a .

Recovery After a Mini Breast Lift

The recovery after a mini breast lift is much quicker than a full breast lift. Most patients can resume light activities approximately three days after the procedure. Any mild discomfort can be easily managed with over-the-counter pain relievers. It is essential to wear your breast garment as recommended by your surgeon. You may have a shower 48 hours after the procedure. Your surgeon will use dissolvable stitches during your surgery, and there is no need to have these removed at your postoperative appointment. You will receive detailed guidance on how to wear your sports bra. Immediately after a doughnut breast lift, your breasts may feel firm and appear excessively elevated. This is entirely normal, and the breasts will eventually soften and settle into a more natural position on the chest. This takes approximately 3 to 4 months, and silicone gel or tape applied to the periareolar scar from 4 weeks post-operatively helps optimise scar quality. Localised breast swelling is common after surgery and takes between 2 and 4 weeks to disappear.

Most patients, after a doughnut breast lift, will be able to return to light work after five days. Avoid upper body exertion for 6 weeks.Your surgeon will advise you on which activities are safe to carry out in the postoperative period. Your breasts may feel slightly tender and sore for the first four weeks after surgery, and you should avoid any direct knocks to the breast during this period. Our nurses will assess you regularly during the postoperative period, and your surgeon will review you six weeks after surgery to review the results of your periareolar mastopexy. A doughnut breast lift will not affect your ability to have regular breast screening with mammograms. The will utilise specific techniques when imaging women who have had breast surgery to take into account the altered anatomy of the breast.

Mini Breast Lift Results and Scarring

The incision for a mini breast lift is located around the areola only — a periareolar scar that typically fades from pink to pale over 12 months. Immediately after surgery it is normal for the skin around the closure to have a slightly puckered appearance as post-operative swelling resolves — this settles to a smooth, flat scar within a few weeks.

The purse-string closure technique used at Centre for Surgery helps minimise the risk of areola spreading over time — a known concern with periareolar closures. A permanent internal suture is placed to prevent long-term stretching of the areola scar.

A doughnut lift a modest, predictable degree of nipple elevation and breast skin tighteningappropriate for the patients it is designed for. Breasts that would need a lollipop or anchor lift to achieve adequate correction will not be well-served by a periareolar technique — this is something your surgeon will assess at consultation.

The mini breast lift is particularly for patients where augmentation alone cannot correct mild ptosis (as nipple position cannot be altered by alone), but where the degree of sagging is too mild to justify the more extensive scarring of a full lift.

Final settled results are visible at 3–4 months as swelling resolves and the breasts settle. Results are long-lasting but not permanent — ageing, significant weight changes, and pregnancy can affect the breast over time.

Breast screening (mammography) is not affected by a periareolar mastopexy. Always inform your mammographer of any previous breast surgery before screening.
